Abstract
This project is an initial phase expansion in the development and set-up of a community “Healthy Aging Initiative” infrastructure to support healthy life styles. This initiative is a comprehensive program that is built on evidence-based information and the need for early intervention in identifying and intervening in “Early Risk” populations, specifically the Baby Boomers. We plan on educating the community and promoting the adoption of healthy habits and life style changes as a strategy to decrease high impact of chronic disease on cost and quality of life. Promoting this project as a community wide initiative, identifying healthcare organizations as leaders acknowledging the influences of social determinants and environmental factors on health outcomes will facilitate collaboration across medical and nonmedical groups and key stakeholders. Project activities and outcomes will be compiled in a detailed reference guide to support working groups across the US succeed in project design and development of similar initiatives.
